Requesting a Birth Certificate

Birth and death certificates can also be obtained online, by mail, or by fax using the information and resources on this page.

Ways to Receive a Birth Certificate

• In person: By appointment only. During the appointment you will need to fill out an application and present a current, valid ID.
• Mail to: Print and fill out the application. Attach a copy of your current, valid photo ID.
  • Send to Larimer County Vital Records, 1525 Blue Spruce Drive, Fort Collins, CO 80524.

  Valid ID Requirements

  • All requests must have a Valid ID submitted with the application. See valid ID list for proper documentation. (English) (Spanish).
  • Important note: Additional documentation may be required as proof of eligibility.

  Cost

  A certified copy of a birth certificate is $20 for the first copy and $13 for each additional copy of the same record ordered at the same time.

  Frequently Asked Questions:

  • The registrant (person named on the birth certificate)
  • Parent/Co-parent: Must be listed on the birth certificate
  • Grandparent/Great-Grandparent: Birth certificate(s) and/or death certificate(s) to show proof of relationship is required (cannot accept baptismal, hospital records, or school records).
  • Siblings/Half-Siblings: Birth or death certificates proving at least one parent in common (cannot accept baptismal, hospital records, or school records).
  • Current Spouse: Certified copy of marriage/civil union certificate is required, or for common law marriage, some examples of documents we can accept are notarized affidavit of common law marriage.
  • Children/Grandchildren/Great-Grandchildren: Birth certificate(s) and/or death certificate(s) to show proof of relationship is required (cannot accept baptismal, hospital records, or school records).
  • Legal Guardian: Original certified court order proving custody is required.
  • If your relationship to the registrant (person named on the birth certificate) is not listed above.

  Larimer County can only issue certified Colorado birth certificates. We cannot issue birth certificates from other states or countries. Please contact the State Vital Records Office in which the birth occurred or the county for further guidance on processes and fees.

  Larimer county cannot make any changes/corrections to Colorado birth certificates. All changes/corrections must be processed through the State of Colorado Vital Records office at 303-692-2200 (cdphe.colorado.gov).

  In order to obtain the certified Colorado birth certificate you must apply at a vital records office. The hospitals only register a child’s birth.

  There is a fee each time someone receives a birth certificate.

  Larimer County also accepts vouchers as payment from House of Neighborly Services, Catholic Charities, and Human Services departments, including other counties in Colorado.

  Larimer County can issue certified birth certificates for all births that occurred in Colorado. You do not have to obtain the birth certificate from the county the birth occurred in.
  

  Our Fort Collins location is the only vital records office in Larimer and Jackson County. Our offices in Loveland and Estes Park do not offer vital record services.
